Hospice At Home
Our Hospice at Home service provides compassionate overnight care for patients who are believed to be in the final two weeks of life, enabling them to remain in the comfort and familiarity of their own home.
The service has been running since August 2010 and is available 365 days a year.Hospice at Home nurses work closely with your GP and District Nursing team, sharing responsibility for your care over a 24-hour period. This coordinated approach allows patients and families to have greater choice about where care is received, and can help avoid unnecessary hospital admissions.
The service is available to patients aged 18 years and over who are registered with a Corby GP practice.

Our Hospice at Home Nurses
All Lakelands Hospice at Home nurses:
- Are Registered Nurses with additional training in palliative and end-of-life care
- Work in accordance with the Nursing and Midwifery Council (NMC) professional standards
- Provide support and reassurance to patients, carers and family members
- Follow Lakelands Hospice Health and Safety policies
- Carry out a risk assessment for every patient and maintain accurate clinical records
How the Service Works
Patients are referred to the Hospice at Home service by a health professional. The service operates overnight from 10.00pm until 6.30am. Our specialist nurses attend the patient throughout the night. Where possible, we aim to ensure continuity of care, with the same nurse visiting the patient each night whenever feasible.

Lynn Davies, Hospice at Home Coordinator, explains:
“Patients are referred to the service during the last two weeks of life. Our nurses provide overnight care and work closely with the District Nursing team to ensure patients receive continuous, compassionate support in their own homes.”
Cathy Finnerty, Lakelands Hospice Manager, adds:
“Unfortunately, dying is a part of living. Though very sad it doesnt have to be bad. We can help patients and families to experience a “good death”.How someone dies remains in the memories of those who love them. It is incredibly important that we get this right..”
